Where are the MHSA Proposal Forms?

but not so fast, which ones do we need, you ask?

Well, you start with Exhibit 3 of course! LOL (instructions are here: http://dmh.lacounty.gov/hipaa/downloads/MHSAProjectProposalProcess.pdf)

At the link above, here are the forms you need to create a full application, and the order I recommend you complete them in:

1. Exhibit 3 - Project Proposal Description (do page one first, then come back to the rest after the budget and risk forms below)
2. Exhibit 4 - Budget Summary (Excel version)
3. Appendix A - Project Risk Assessment (fill this out before you go to page 2 of the #1 item above, Exhibit 3)
4. Exhibit 3 - Project Proposal Description (now finish this one from above)
5. Appendix C - Summary Project Schedule
6. Appendix C-1 - Detail Project Schedule
